Nurse-Family Partnership
Nurse-Family Partnership provides mothers with support in parenting their children from pregnancy through the child's second year.
Who is eligible for this program?
This program is available to first-time mothers who enroll during pregnancy. Women must live in La Plata, Archuleta, San Juan, Montezuma or Dolores Counties and meet financial eligibility guidelines.

Home Visit Experience
Registered nurses, together with Nurse-Family Partnership clients and their families, engage in activities associated with the three Nurse-Family Partnership goals during each home visit.
