Nurse-Family Partnership Program Manager
Wage Range: $41,925 - $48,750 for 1950 hrs/yr.
The position provides supervision of the nurse home visitor staff, data entry and has responsibility for program referrals, budget expenditures, meeting program objectives and outcomes, and community outreach as well as responsibility to serve families with home visits.
Education: Requires a Bachelor of Science in Nursing from accredited college or university. State of Colorado nursing license. (Masters Degree in Nursing and experience in OB/GYN and Peds preferred) Public Health and home visitation experience a plus.
Job responsibilities include the following:
- Program Management 50% and home visitations 50% of program time.
- Attend NFP-specific education, including supervisor-specific education.
- Position currently supervises 3 nurse home visitors.
- Use program reports to assess data and manage areas for program quality enhancement.
- Meet one-on-one with each Nurse Home Visitor at least weekly to provide clinical and reflective supervision
- Conduct at least two team meetings per month: discuss program implementation and case conference to identify client problems and solutions.
- Invite experts from other disciplines to participate in case conferences.
- Make a minimum of one home visit every four months with eachnurse.
- Network and participate in community based coalitions.
- Develop and convene a Community Advisory Board.
- Maintain relationships with key community partners and Departments within the Health Dept, a Medicaid provider for children.
