Obtaining a Death Certificate

Death certificates are confidential and can only be given to blood relatives.

Death certificates can be released to: the mother, father, children, brother/sister, ex-spouse for Social Security benefits, or the title company - if they have a letter stating the document will stay confidential.

NOTE: Death certificates are issued only from the county in which the death occurred.

If you wish to get a death certificate through San Juan Basin Health, you can download the attached form below, fill it out and bring it to our office with the proper identification (listed on the form). You must present identification or a driver's license.

Certificates cost $17 for the first one, $10 for the second copy per search.

Please note: San Juan Basin Health only accepts cash, check or money order.

You can come into the San Juan Basin Health office between 8:30 and 4:00 or mail a $17 check or money order with a photocopy of your identification and a self-addressed stamped envelope to: San Juan Basin Health Department; Attn. Vital Statistics; PO Box 140; Durango, CO 81302

or for Archuleta County: You can come into the San Juan Basin Health office between 8:30 and 4:00 (located at 502 S. 8th Street in Pagosa Springs) or mail a $17 check or money order with a photocopy of your identification and a self-addressed stamped envelope to:San Juan Basin Health, Attn: Vital Statistics; P.O. Box 597; Pagosa Springs, CO 81147
