Childhood & Adult Immunizations

We provide routine immunizations for children and adults by appointment only on Mondays between 10:30 and 5:30. Please call 247-5702 during our regular business hours (8:30 to 4:30) to schedule an appointment.  

Pagosa Springs: Immunizations are available on Mondays at our Pagosa Springs office between 2:00 and 5:15 p.m. by appointment. Call 264-2409, ext. 0 for an appointment.

Payment: We accept Medicaid, Rocky Mountain HMO, CHP+. Cards must be presented at the clinic at the time of the visit.
