Children with Special Needs
The Health Care Program for Children with Special Needs (HCP) is a resource for families, health care providers and communities. The program focuses on care coordination and developing more efficient and accessible systems of service for families with children with special health needs.
HCP serves children and youth from birth to age 21 that have, or are at risk for, physical, behavioral or emotional conditions. HCP promotes a medical home team approach to assure coordinated and comprehensive care for children with special needs.

HCP Services – "Connecting Kids with Care"
Public health nurses or social workers, along with other health care team members, provide care coordination to families. Other HCP staff may include a family coordinator, dietitian, audiologist, speech-language clinician, and occupational or physical therapist.
HCP Care Coordinators assist families:
* identify a primary care provider
* identify members of their health care team
* identify sources of payment for needed health care services
* understand eligibility requirements for program assistance and community resources
* access pediatric specialty care
* access HCP Specialty Clinics in rural areas of Colorado (Neurology, Cardiology, Orthopedics, Rehab)
* access HCP Diagnostic & Evaluation Clinics (pediatric developmental and diagnostic services)
* facilitate a medical home team approach – to assure coordinated medical services and access to community resources and family supports
HCP Care Coordinator’s knowledge and skill includes:
* identifying and accessing community services beyond the health care arena
* assisting with follow-up for newborn metabolic and hearing screening
* promoting the use of developmental screening tools
* identifying and referring for vision and developmental screening resources
* connecting families with support groups and family leadership opportunities
* collaborating to develop systems that are easy for families to use
* providing culturally responsive services
Eligibility: HCP Care Coordination is available for all children and youth with special health care needs and their families regardless of income.
