Childhood & Adult Immunizations
We provide routine immunizations for children and adults by appointment only on Mondays between 10:30 and 5:30. Please call 247-5702 during our regular business hours (8:30 to 4:30) to schedule an appointment. Pagosa Springs: Immunizations are available at our Pagosa Springs office by appointment. Call 264-2409 for an appointment.
Immunizations are NOT just for kids! Whether a young adult, middle-aged adult, or senior citizen, we ALL need immunizations to keep us healthy. Read more about adult immunizations and a chart of recommended vaccinations. Take a short CDC quiz to find out what vaccinations you might need!
Please bring records of previous immunizations. Please bring records of previous immunizations or for faster service, fax them to 247-9126 in Durango (and fax to 264-4640 in Pagosa) and we’ll prepare ahead of your appointment. Be sure to include Name of Child, Date of Birth and Parent Name along with a Phone Number where we can reach you.

San Juan Basin Health is a provider for Vaccine For Children (VFC) program – Federally funded to provide vaccinations to children regardless of ability to pay; the vaccine products are provided to you at low or no cost. Your child/ren age 0 through age 18 may qualify based on these criteria: 1) Medicaid; 2) CHP+; 3) Native American/Native Alaskan 4) No Health Insurance coverage; 5) Under-insured (health insurance does not cover vaccine services or you have not yet met your deductible) A $14.70 fee per vaccine helps supports the costs of providing the free vaccine products and is considered a donation. No child is ever turned away for inability to pay.
Payment: We accept Medicaid for children, Rocky Mountain HMO, CHP+. Cards must be presented at the clinic at the time of the visit. We also accept cash or checks; but no credit cards at this time.
