San Juan Basin Health Department is joining other local public health agencies across Colorado in developing a Public Health Improvement Plan. This process began with collecting health data and input from community members and stakeholders. A Community Health Assessment for Archuleta and La Plata counties was completed in June 2012. From here, an internal process in the health department and external process in both counties guided the selection of two health issues to be addressed in the Public Health Improvement Plan (PHIP).
