Birth or death certificates may be obtained either through San Juan Basin Health Department or directly through the Colorado Department of Public Health & Environment. Individuals can come into San Juan Basin Health’s Durango office for birth and death certificates: 8:00 a.m. to 3:30 p.m or to the Pagosa Springs office from 8:30 a.m. to 4:00 p.m. For the Durango office, applications will be accepted and certificates will be ready on the next business day at 10:00 a.m.
Obtaining a Birth Certificate
Birth certificates are confidential and can only be given to relatives or legal guardians/representatives. Birth certificates can be released to:
- the mother
- the father – only if is listed on the original birth certificate
- maternal grandparents (mom’s parents–proof of relationship required)
- paternal grandparents (father’s parents only if he is on original birth certificate–proof of relationship required)
- the brother/sister (certified birth certificate required proving relationship)
If you wish to get a birth certificate through San Juan Basin Health, you can download the attached form below, fill it out and bring it to our office with the proper identification (listed on the form).
You must present valid identification (driver’s license, etc).
Certificates cost $17.75 for the first one, $10 for the second copy per search.

Obtaining a Death Certificate
Death certificates are confidential and can only be given to blood relatives.
Death certificates can be released to: the mother, father, children, brother/sister, or ex-spouse for Social Security benefits.
NOTE: Death certificates are issued only from the county in which the death occurred.
