Communicable Disease

All agencies are required to track the incidence and distribution of disease in the population and prevent and control vaccine-preventable diseases, zoonotic, vector, air-borne, water-borne and food-borne illnesses, and other diseases that are transmitted person-to-person.

We serve Archuleta, Dolores, La Plata, Montezuma and San Juan counties and provide support to the Southern Ute and Ute Mountain Ute tribes.

Collecting disease information

We collect and report disease information according to Colorado Board of Health Rules and Regulations.

Investigating cases of reportable disease

We investigate cases of reportable diseases and suspected outbreaks according to standard protocols and guidance provided by Colorado Department of Public Health and Environment (CDPHE).

  • Our Epidemiologist can be reached at (970) 335-2001 and leads the Communicable Disease Response Team that provides 24/7 on-call disease reporting. For after-hours response, please call our main number (970)247-5702 and select option 1.
Immunizations

We provide routine immunizations for children and adults and in collaboration with Colorado Department of Public Health and Environment (CDPHE), monitor community immunization levels. San Juan Basin Health also provides international travel vaccinations.

  • We use the Colorado Immunization Information System (CIIS) as a tool to document an individual’s immunization history and record vaccines received. If you need an immunization record for yourself or your child, please call (970) 335-2013. There is a $5.00 fee.
  • Routine immunizations for children and adults (Durango & Pagosa Springs)
    • Durango (970) 335-2013 call Mon-Thurs 8:30-5:00, Fri 8:30-1:00; appointments available Wednesdays and Thursdays.
    • Pagosa Springs (970) 264-2409 call Mon-Fri 8:30-4:30, appointments available first Thursday of every month.
    • If you are new to our Immunization clinic, please bring your vaccination records with you to the appointment.

    • We are a Vaccine for Children (VFC) program that is federally funded to provide vaccinations to children regardless of ability to pay. Your children ages 0 through 18 years may qualify based on these criteria: 1) Medicaid/eligible; 2) Native American/Native Alaskan 3) No health insurance coverage; 4) Under-insured (health insurance does not cover vaccine services or there is vaccine cap).

    • Adults who qualify (see above criteria) may also be eligible for low cost vaccines.
    • We accept credit cards, cash or checks along with Medicaid insurance plan.
Travel Vaccinations

San Juan Basin Health provides immunizations for international travelers by appointment.

  • Please schedule your appointment 4 -8 weeks prior to travel and bring your vaccination records with you to the appointment.
  • Durango (970) 335-2013, call Mon-Thurs 8:30-5:00, Fri 8:30-1:00; appointments available Wednesdays and Thursdays.
  • We accept credit cards, cash, or checks. A receipt is provided for you to submit a claim to all insurance.
  • Appointments consist of reviewing traveler’s immunization history, destination health risk counseling, and vaccination recommendations, as well as administration of vaccinations. A yellow “International Certificate of Vaccination” is issued at time of vaccinations.

Community-based disease prevention and response

We take appropriate measures to prevent disease transmission using methods specific to: infected persons (isolation, treatment, contact tracing/notification); contacts to infected persons (quarantine, prophylaxis); and the environment in which the communicable disease occurs (facility closure, disinfection).

  • Our Epidemiologist (EPI) is involved with our Environmental Health staff for some water (e.g. swimming pool), food (e.g. point sources such as restaurants) and vector-borne (e.g. insects, rodents carrying plague, rabies or typhoid) communicable disease responses. To report an animal die-off or suspicion or a diseased rodent, please call our EPI at (970) 335-2001 or our Environmental Health staff.
  • Our Epidemiologist is a subject matter expert during public health emergencies related to communicable disease, working closely with Emergency Preparedness and Response staff by leading our internal Public Health Incident Management (PHIM) team and participating in the La Plata, Archuleta, San Juan, and Southern Ute (LASST) Healthcare Coalition.
Regional cooperation

We work closely with CDPHE in communicable disease investigation and control, particularly if the investigation crosses county lines or technical assistance is needed.
