San Juan Basin Health provides an Options for Long Term Care program that offers services which assist elderly and individuals with disabilities in staying in their home as long as possible. This is a Medicaid program and the first step is to apply for long term Medicaid through Department of Social Services. This program offers individuals and their families alternatives to nursing home care.
Our Single Entry Point (SEP) case managers assess clients to determine if they are functionally eligible for the program and if so, can assist clients in getting help with homemaking, personal care, non-medical transportation, home modifications, and respite care.

Medicaid Service Programs for Older & Disabled Adults
SERVICES:
- Homemakers assist individuals with household management tasks such as cleaning, shopping, meal preparation, laundry, and linen changes.
- Personal Care Providers assist individuals with tasks such as bathing, hygiene, hair care, and nail care.
- Alternative Care Facilities provide 24 hour supervision and care and are considered by Medicaid to be an alternative community setting versus a skilled nursing home level of care. Assisted living care also includes meals, laundry, cleaning, transportation, and some personal care.
- Electronic Monitoring offers a range of services from a button that can be worn and pushed at any time a person needs emergency assistance, to fall monitors and medication management systems.
- Home Modification services provide needed modifications to one’s home to accommodate a disability. An example might be a wheelchair ramp or wheelchair accessible shower.
- The Consumer Directed Attendant Support Program is the newest service offered under the waiver programs. It allows recipients to benefit from homemaker, personal care and attendant services from a provider chosen by the recipient. The recipient receives an allocation based on his/her needs, and the individual or their representative manages the necessary care within the allocation.
- Our agency is also involved in coordination of Medicaid skilled long term home health benefits, such as nursing and certified nurse assistants.
